On May 16, shortly after 1:30 pm, Ortega arrived in the gym armed with an Olympic PCR-223 caliber rifle and Multiple ammunition magazines, LVMPD Asst. said sheriff Jamie Proser.

Ortega walked around several people who left the gym and approached an employee who worked at the reception. Proser said Ortega then began to “tension” around the reception as several other people left the business.

A second employee approached Ortega and the two “briefly exchanged words.” The employee escaped after Ortega lifted the rifle and started shooting, hitting the employee.

Ortega followed the employee more deeply in the business, continuing to shoot before returning to the front lobby where he continues to shoot, Proser said.

At some point during the incident, Ortega’s weapon does not function, Proser said, although he did not specify how. She later said that this malfunction “saved the incident from being an event for widespread intelligence.”

Around 1:35 pm, several people called 911 to announce what was happening, and the officers were sent to the gym.

“I think I saw someone enter with AR,” said a man who called 911.

Moments later, the conversation shots were heard.

At 13:39 Officers Kaed Urban, Skitter Black and Aaron Javier arrived in the gym. All three officers approached the front doors and saw Ortega.

Officer Javier fired a circle out the window in Ortega. Shortly thereafter, Ortega left the gym and tried to walk past the officers.

The Urban police officer shot four rounds, and officer Black fired one, hitting Ortega.

The police then took Ortega in custody and began to provide life -saving measures. The medical staff arrived and took Orto to a local hospital, where he later succumbed to his injuries and was declared dead.

Additional officers arrived and found Quinonez, as well as three other victims suffering from firearms. The medical staff took them to local hospitals.

Later, police learned that another person had taken another hospital with life-threatening injuries.

Proser said during a briefing that on Monday afternoon, police could not confirm anything about the motive as the investigation is still ongoing.

She said Ortega was a member of the gym and worked in this place, but there was no connection between the victim and the suspect.

Ortega would have encountered the following accusations if she had survived:

  • One count of murder with a deadly weapon

  • Three pcs for attempted murder

  • Three attacks with a deadly weapon of a protected person

  • Five attacks with a deadly weapon

This incident marks the third firing related to the officer in 2025 and the third fatal. At that time, in 2024, the subway had three shootings related to officers, with three casualties.
